Personal Data Request Form

This form is to support a “verifiable consumer request” for personal data.

That is defined as “a request that is made by a consumer, by a consumer on behalf of the consumer’s minor child, or by a natural person or a person registered with the Secretary of State, authorized by the consumer to act on the consumer’s behalf.

In addition:

  • You may make a personal data request twice within a 12 month period.
  • The request must include sufficient information that allows us to verify you are the person we will assess the data request for.
  • Describe your request with sufficient detail that allows us to properly understand, evaluate, and respond.
